Console should have server version and ip name in HTML title

Right now the browser title for Geronimo console is "Geronimo Console"
This should be changed to "Geronimo 2.1.3 Console @ myserver.mydomain"
If you have open consoles for different versions and on different servers, then
windows toolbar tooltips display this information.
