Just investigated the icon of restart item. It is contributed by
org.eclipse.wst.server.ui, no icon is specified by this plugin. After checking
the popupMenus in eclipse, it is common that no icon is provided for the
popupMenu.
As to the second issue listed above (i.e., restart item is enabled when the
module is in stopped state), it is also common the server adapter.
I will close this JIRA.
