Now I use Far manager for writing/editing, and MSVC for debugging. Some time ago I used MSVC 6 and MSVC 2003 with VisualAssist - it's really great plugin! AFAIU MSVC 2005 includes VisualAssist features; I also heard a rumor that WholeTomato team was engaged for this :)
