HAWQ-1253 - monitoring section add xref to hawq_toolkit
reviewed the page:
- moved hawq_toolkit section from near end to near front - it was
referenced multiple times before its discussion
- added a link to hawq_toolkit reference section
- reviewed and updated examples to make sure they work
- multiple wording changes
