So I tested 4.5.2 with the test resource at
<https://nghttp2.org/?103-eh>, and the code appears to work as designed
(good!).
That said, how about (optionally) exposing 1xx messages to the caller
through a handler interface?
(Is anybody working on this? Will it be in 5.x? Would patches be accepted?)
