how can pre_connection hook distinguish between client (inbound) connection and proxy (outbound) connection?

I am not aware of a reliable mechanism at present, but I would greatly
appreciate any hints.

If there is not an existing mechanism, I'd like to add something prior
to 2.2 APIs being cast in stone.
