Seen this both in production and prototyping mode. Prototyping mode allows to
refresh the meta model, which fixes any missing action buttons. With production
mode, one needs to restart the App. Though seen very rarely, this is mission
critical.
My assumption is, that we have a race condition when introspecting the domain
classes.
