Or maybe it isn't fixed?

[~andy.seaborne] {{ResultSetMgr}} has no class initialiser and it goes via 
{{ResultSetReaderRegistry}} which does its own class initialisation but doesn't 
call into system wide initialisation.  I know we can't necessary cover every 
possible entry point into the system with initialisation but maybe this is 
another place that should do this?
