Add the token exchange constants, implement the TokenIdentityHelper (canonical
identity extraction), add an act chain depth counter, and wire the
grant_type=token-exchange dispatch into TokenResource.doPost() with the
same-subject exchange path fully functional. Cross-subject and interactive
delegation are added in a later task; this task provides the foundation and a
testable same-subject flow.
