the {proxy-url} input module
java/org/apache/lenya/cms/cocoon/components/modules/input/ProxyUrlModule.java
should be removed, for three reasons:
* it is currently unused and hence untested and unmaintained
* it uses the deprecated document-id instead of uuids
* it is wrong for high-level pipelines to be concerned with proxying.
proxy url rewriting should be centralized and not be part of any
publication-specific or module-specific sitemaps. hence, the lenya api
should not visibly present a proxy-url input module.
