I had a nightmare with host header problems.
Someone here told me about a check box in the admin area which has to be checked/unchecked if you are running several hosts on the same server. I'm not sure if this has anything to do with your problem but it might be worth a go ....

Type www.domain.com and you see the coldfusion source code for the index.cfm
Type www.domain.com/index.cfm and everything works fine
Type domain.com and everything works fine
I only ever had this once before, and I don't think I ever figured out the cause last time either.
Baffled?
Any one else ever seen this.
