Well I solved this problem and it is in fact nothing to do with CF at all.
So Macromedia's technote really is just a crappyworkaround.
For anyone that is interested, I have blogged he solution.

I have this problem on IIS5 with Coldfusion5.

Just started having it on a server that was fine before, although the disk
has been transplanted to a new server.

Anyone got any idea why suddenly CF would need the application protection to
be set to LOW when it worked fine as MEDIUM before?
Of course the other problem is that sites will default to medium, there
seems to be no way to make them default to LOW when they are created.
