But google will index PDF content which is really cool. You cannot really download flash paper as many users will not be able to use it once they have the file, as unless you have the flash desktop player installed you can only view it by dragging it into the browser window. So if you want to offer a downloadable version I would go with PDF. Why don't you get them to fill the form in online, saves a lot of hassle!
