YES! With CF, you can read the source PDF file with DDX DocumentText. So you can parse through it and add hyperlinks to them, then CFDOCUMENT to a new PDF. You lose formatting though and have to recreate and reformat the text to recreate the PDF. http://pdfutils.riaforge.org/ can help you get text from the PDF file with getText() If you have LiveCycle, this becomes easier as you can edit the PDF directly. The DDX element LINKS is the tag that would create the link for you. LiveCycle would be able to do this.
