No, it's not available for download, although please do mail me (probably off list?) if you're interested in using it.
However if you want more details on integrating it to a site I can give them to you. Assuming you have the code for it installed on your server, basically in order to link to a form you need to pass the parameter "formid" to a page which makes a call to the formbuilder custom tag which takes the formid as a parameter, and that's pretty much it... when a user submits a form it submits to itself and all data capturing / processing is handled by the custom tag.

We have an online form builder you could take a look at:

This is written in Cold Fusion and was built to integrate with our CMS, but would easily work as a stand alone package.
This includes validation and also a simple statistical analysis of user responses via a published form.
There is a sample (non-editable) form in the demo to explain what each of the elements does, but otherwise you can create and edit new forms as usual (obviously there is no site configured to publish them to).
Have a play around with it and if you have any questions then ask away!
