Just as a clarification, regardless if PAM_TTY is set for example to "tty2", or just not set at all, things don't work. So it must be set exactly to "tty1" for things to work...
We were originally using "tty7" which is the first free one and were wondering for long time what is the culprit.
After lengthy comparison with user-session-launch source code we noticed that it has PAM_TTY=tty1 hardcoded and finally we tried that in tlm.conf and things started to work... This detail certainly wasn't our primary suspect...
