Attached patch with a simpler fix for 3.0/2.9.

IndexWriter and IndexReader now pass the synced set to IndexFileDeleter, which 
in turn removes files from it once they are deleted.  This means the set can 
only be as big as the number of sync'd but not yet deleted files in the index.
