Rob,
I see what you mean: ngroups should be the same as the number of groups. This
problem still happens when I don't request highlighting, so it appears to be a
problem with grouping only.
This may be related to a known issue - from the {{group.ngroups}} description
on [http://wiki.apache.org/solr/FieldCollapsing]:

documents that belong to the same group have to be located in the same shard,
otherwise the count will be incorrect. If you are using SolrCloud, consider
