This patch adds a new PATCHED_TABLE implementation to the norms format.  It 
also does the following:
* Renames PATCHED to PATCHED_BITSET
* Refactors NormMap to allow reassigning ordinals in descending order of 
frequency for easier statistical analysis of top values.
* Reduces memory usage of TABLE_COMPRESSED by restricting using a byte[] and 
limiting any docs iteration containing values outside a byte range to 
DELTA_COMPRESSED.
