Looking at the catch of ExitableDirectoryReader.ExitingReaderException in
SearchHandler, it resets numFound to 0, can add a duplicate "response", and
overwrites any existing "debug" section with an empty debug.
Dummy entries should only be created if the real entries don't already exist.
