The {{[horizontal]}} definition list ends up in a html table with keys in a
{{<th><code>foo</code></th>}} tag. The text here is white on white background,
since it inherits from the {{table th code}} rule in {{customstyles.css}}
A possible fix is to set black bold in ref-guide.css, see PR.
