Committed a fix to SegmentReader (r1529611) to clean unused memory
(un-referenced DVPs) as well as don't use an anonymous RefCount class since it
resulted in a memory leak, where those anonymous classes held a reference to
their SR, therefore we always referenced unused DVPs.
