This is a very old problem. I did a fair bit of investigation on it and added
a detailed diagnostic mode you can configure in properties.xml, which will
basically track all transactions and when it happens tell us what else is going
on. The diagnostic setting does not work, however, in a cluster environment,
and in any case I had enough information previously provided to conclude we
were seeing transactional integrity issues with both Postgres and some versions
of MySQL.
So what I am most interested in is your database configuration at this point.
