I would also recommend soliciting templates and example forms from conference participants. It is one way of engaging people and getting them interested in OOo.
Right off the bat, I can think that mailing labels for envelopes will be nearly universal for public libraries. I defer to the expertise of others on the topic of adhesive labels for the specifics.
