I'm not very familiar at all with the pdf plugin, but I'd like to suggest renaming it
to address the source file format, not the target output format.
The 'latex' plugin, for example, can produce PDFs also, but from LaTeX sources.

Description:
I have completely re-written the PDF plug-in. It solves most of the thorniest issues,
such as table formatting, more levels in the table of contents, and image sizing. The
graphic design is also dramatically improved.
