I'm not sure fop will be the best candidate for this. It will be the most simple because we already have templates but I'm not sure it's the most simple to use. There are some libraries on the net for read/write pdf with java code, I don't know if there are some with Apache compatible license.
I am familiar with embedding fop in Java so that's something I would try first. The fop team expects (unofficially) another release 'in the second half of 2005', I am eagerly awaiting that.
As alternative, iText comes to mind ( http://www.lowagie.com/iText/ ), it has a dual MPL / LGPL license. It looks good but I have never used it myself.
