Fix For: 0.19.0
Currently we will allow subsequent operations following a failure in the
Registrar. While this is safe, it is a bit tricky to reason about.
Instead we could transition to a failed state, in which any incoming operation
is failed. This is the model followed by {{Group}}, {{StatusUpdateStream}}, etc.
