Completed impl for PreJsf2ExceptionHandlerFactory.
Changed PhaseListenerManager and LifecycleImpl to publish exceptions instead of
logging them.
Did not do anything with ExceptionHandlerFactory since there is an open item
for that (really just need to get it registered with the FactoryFinder).
Also, the spec seems to indicate that there are a lot of places where
exceptions should be caught and published. I'm not sure what that all
involves, so I left it alone for now.
