The reason for (2) is best explained with the following Java code fragment:

String url = "file:///path/index.html";
URL u = new URL(url); // o.a.n.protocol.file.File, line 95
url = u.toString(); // url is now file:/path/index.html (with OpenJDK 1.7.0_65)

The question which form is canonical seems to be answered by Java:
{{file:/path/file}} (one slash only). We can hardly use another one because
there are many URL <=> String conversions.
