Patch!
* adds indexer.delete.skipped.by.indexingfilter parameter defaulted to true
* adds param to nutch default plus missing other param
* moved indexing filters to the bottom so it can have control over all fields
that were added by indexermapreduce (only the boost was not adjustable by a
filter)
