rename faultType into faultName whereever it makes sense.
org.apache.ode.bpel.iapi.PartnerRoleMessageExchange#replyWithFault is a good
start point.
rename table column BPEL_MESSAGE_EXCHANGE.FAULT_TYPE into
BPEL_MESSAGE_EXCHANGE.FAULT_NAME (see HMessageExchange)
