The problem is that mysql server was not configured to accept a packet of data
of specified size.
To fix the problem:
1)need to modify mysql's my.cnf, max_allowed_packet to a value larger than the
size of uploaded file.
or
2)start mysql server with the following: mysqld
--max_allowed_packet=some_larger_value
