Order item quantity input fields do not appear for new ship groups added to
existing orders because editorderitems.ftl uses the OrderItemShipGroupAssoc
entity for controlling the display of quantity fields. editorderitems.ftl
should be changed to use OrderItemShipGroup for displaying quantity fields so
that quantities can be associated with new ship groups added to existing orders.
