The 'request-confirmation' and 'confirmation-message' menu item attributes
were available prior to the recent widget refactor, but they were only used by
the HTML renderer (not the macro renderer). So, I removed them.
I will put them back in the menu widget models, but someone else will need to
update the renderers to use them.
