Same bug as OPENJPA-3024, but in `orm.xml` and older: `orphan-removal="true"` together with `<cascade-remove/>` or `<cascade-all/>` lost the orphan removal, because `XMLPersistenceMetaDataParser` applies orphan removal when the element starts and `finishField()` → `setCascades()` overwrites it. Orphan removal is now applied after the cascades in `finishField()`. The 4.2 migration note is reworded to describe the behaviour for annotations and XML. `TestOrphanRemovalXml` (one-to-many and one-to-one, cascade-all and cascade-remove).
