I will give it a try...
Push-to-talk should work like this (my understanding):
There need to be a button (most likely configurable), that need to be
pushed while a given user wants to say something. After release this
user is silenced.
Therefor choose devise and establish stream has to be done in the first
place (room entry). This is more like an "Unmute while push it"-key.
This is useful for users without headset and probably to get a bit more
discipline during a session.

Could you please describe how this button should work from the user point of

Currently we have "Exclusive audio" mode, maybe this mode is what you need?
