In my implementation, HEPATOSPLENOMEGALY has only one synonym (HSM) and HOLOSYSTOLIC MURMUR has two synonyms (MURMUR HSM and HSM). None are set as "preferred". Is one assumed to be preferred by the API if none are marked?
