The best way to find the root cause is to go to the concept edit page and try saving any of those concepts, if they get saved without that error, then they are fine and implies there is something wrong happening in the module which causes it to re-export a concept otherwise get rid of the duplicate names.
What resultset do you get when you execute the following sql query? select c.concept_id from concept c, concept_name cn where c.concept_id=cn.concept_id and cn.name = 'DERMATOPHYTOSIS'; You can test it for all the concepts with issues.
