To get custom hl7 processing in openmrs the fastest you would want to
create a module and add a handler there that extends/overrides the oru_r01
processor.  See https://wiki.openmrs.org/display/docs/Custom+HL7+Handlers

Would it be possible to add something for admins to customize the processor?

Perhaps a GP that contains a list of PersonAttributeType/pid segment pairs?
