There is a report on the Forum (and I confirm) that PDF import extension 2.1.0 is not working on OO 4.0.1. The earlier version 0.1.0 is working. The extension site entry for 2.1.0 says this is identical to 0.1.0 apart from changed ID and version number.
I've had a quick look inside the two packages; the difference seems to be in description.xml, where a chunk of <update-information has been omitted. I assumed that the main files of the same date (roughly July 2013) would be identical - I may be in error.
