I determined the problem... My database user did not have permissions set properly. I downloaded a UK extract to test out and was able to correctly render the London area.
I have also updated the Mapnik wiki page under generate_tiles.py and
generate_image.py to inform others about this possible pitfall (albeit I should
have figured it out earlier!)
