there was a little edit war recently where one mapper put maxheight on a river node under a bridge, and another mapper removed that again on the grounds that JOSM used a road traffic sign icon to display it (which made no sense on water).

Is it possible to say "use this icon for maxheight, but only when the way on which the node sits is tagged highway=*"?

Yes that is possible. We simply need to define a rule consisting of two conditions. There are some examples included in default style. Most active user of this is the FreieTonne style.

On a general note, are the (AFAICS German) road signs that JOSM uses in its default style a good idea for an editor that is used (hopefully) the world over?

We need to choose one set, and as the majority of developers and users is German, we use these. There are alternative styles (e.g. for GB), but it seems interest is low. I think most users understand the idea of abstraction and simply accept any symbol as long as it makes sense.
