Summary: Migrate issue URLs

There are references to github issues in the code. Since we migrated to jira,
those URLs are not valid. This task is to convert all issues to the new format.
For example, https://github.com/julianhyde/optiq/issues/127 becomes
https://issues.apache.org/jira/browse/OPTIQ-127.
Note that pull requests are still valid. URLs such as
https://github.com/julianhyde/optiq/pull/227 must not be changed.
