For anyone else who may suffer this issue. Cloudflare looks to be changing the s-maxage itself for some reason. I am looking into the issue but unrelated to Pagespeed. ModPagespeedDisallow "/banners/" would mean no s-maxage header is set. By default cloudflare does not store .mp4 files which marks it as DYNAMIC.
