Hopefully we can figure out what needs to be done and take care of several 
issues at once.  I'd suggest specifically looking at PDFBOX-166 which says 
"I had to inverse the K value..." and "I hope this is correct."  So that 
might be a place to look to see if that is causing or will solve the 
problem.

On a side note, I think PDFBOX-89 and PDFBOX-260 are done but I haven't 
confirmed this (I don't do much with images in PDFs).  Other issues listed 
above may also be fixed, as many of them are rather old.

The problem seems to occurs when colorspace of the PDPixelMap class is a
instance of PDIndexed.

pdxObjectImage.write2file(new File(fileName));

 

for a png file, it gets a distorted image in a lot of cases, is there some
parameters that I need to set when using this method?
