Hopefully we can figure out what needs to be done and take care of several issues at once. I'd suggest specifically looking at PDFBOX-166 which says "I had to inverse the K value..." and "I hope this is correct." So that might be a place to look to see if that is causing or will solve the problem. On a side note, I think PDFBOX-89 and PDFBOX-260 are done but I haven't confirmed this (I don't do much with images in PDFs).
