@David: I set up the *free* version of jpedal 
http://sourceforge.net/projects/jpedal/ and I get the same (bad) image as with 
pdfbox. This is not surprising, because they use JAI too. Did you use the 
commercial version of JPedal?

(Update: I tried again using their own version of jai 
http://www.jpedal.org/PDFblog/2011/03/java-jai-image-io-jpeg2000-memory-leak-fix/
 and I still have the bad image )
