I solved the problem with the Vattenfall file, the cause is similar to 
PDFBOX-1830: a wrong initialisation. Rev 1571581 had incorrectly changed the 
initialization of lineCap from 0 (= CAPBUTT) to CAPSQUARE. The spec tells "A 
code specifying the shape of the endpoints for any open path that is stroked 
(see “Line Cap Style” on page 216). Initial value: 0, for square butt caps." I 
changed it back to 0 (CAPBUTT) in rev 1580201 and the vattenfall file is now OK.
