Resolution: Duplicate
I have reverted my last change, because it doesn't work for PDFBOX-2282, the
"new, correct" solution is to use the appearance rect (the spec is open to
intepretation). I'm also closing this one (Sorry John) as duplicate of
PDFBOX-2283, where I will bundle my efforts on rendering annotations /
appearance streams.
