Lets try different things.
- Here's page 48 in a single PDF (your p47 is counted from 0, and PDFSplit
counts from from 1). Does the effect happen?
- Please try the 1.8.7 snapshot, get it here:
http://repository.apache.org/content/groups/snapshots/org/apache/pdfbox/pdfbox-app/1.8.7-SNAPSHOT/pdfbox-app-1.8.7-20140830.170955-123.jar
then do java -jar pdfbox-app-1.8.7-20140830.170955-123.jar PDFToImage filename
- also try older versions.

try 1.8.5 down to 1.8.0 to see whether it still happens
- try the unreleased 2.0 version:

the command line app syntax is as usual, but the API is different, so try only
the command line.
