PDFBOX-1234 and PDFBOX-1550 describe such an issue. The DA entry of the form
field has a name which is neither matched in the DR entry of the field nor the
DR entry of the AcroForm. Instead the name matches the fonts base name in one
of the DRs font resources. Please reopen
